2024 Community Health Assessment
Community health assessments collect a variety of information including prevalence of health behaviors, social determinants of health, environmental factors, and community input. The data is analyzed to discover gaps, disparities, and trends happening within the community. The assessment findings are shared with the community to help drive health investments and creation of health programs.
