Medications are becoming harder to understand. With newer medications that are hard to pronounce, hard to take, and hard to pay for, education for patients is important to make sure the medications work. Many people don’t understand their medications because of age, race, education, or low income resulting in low health literacy. Talking with a pharmacist is often the last opportunity patients have to understand medications. HFM Pharmacy has multiple programs to assist patients with their health literacy.
Comprehensive Medication Reviews (CMR’s) are performed between patients and pharmacists to review all aspects of the patient’s medications. The pharmacist educates the patients on proper administration, reviews side effects and drug interactions, explains what the medications are for and how they work, and often reviews for more cost effective options.
Medication Synchronization (Med Sync) helps patients get all their medications filled at the same time. The pharmacy fills the patient’s medications and fixes any problems ahead of time. The pharmacist calls the patient to review any problems and explains that their monthly medications are ready.
Med Sync and CMR programs are just some of the tools that HFM Pharmacy uses to help patients understand their medications and get more bang for their buck.
